Privacy policy
The privacy of personal information is very important in light of the use of technology for psychotherapy services. I am committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for services provided. Please read on to better understand my Privacy Policy.
What is Personal Information?
Personal information includes identifiable characteristics such as gender, age, home address or phone number, education and training, family circumstances or names, health history, health conditions, health services received, occupation/profession, ideas and concerns expressed by an individual, etc.
Reasons why Dr. Anne Bailey Psychology collects Personal Information
Personal information is only collected in order to provide psychotherapy services effectively and in a customized manner to the individual client(s). We would only disclose a client’s personal information without consent in emergency situations or if required by law. Client credit card information is collected for invoicing purposes. The College of Psychologists and Behavioural Analysts of Ontario (CPBAO) may decide to audit me as a professional as part of their regulatory duties to protect the public interest and would have access to personal client information as a result.
Protecting Personal Information
No personal information is stored in hard copy at Dr. Anne Bailey Psychology. Digital records are kept securely in the Jane client EMR platform, which require dual authentication to access.
Limits to the Protection of Personal Information
There are limits to the protection of personal information which are the agencies or parties involved in the protection of minors and vulnerable individuals or legal proceedings (i.e., Children’s Aid Society, authorities involved in protecting seniors from abuse, court orders to release information, etc.)
Retention and Destruction of Personal Information
I am required to retain client records for a minimum of 10 years after the last contact for clients older than 18 years of age and for 10 years after an adolescent turns 18. Personal information collected that is not part of the permanent client record is destroyed securely.
Accessing Your Personal Information
You have a right to access your personal information that I have on file. You can reach out and request access to your client record and I will support you in fully understanding the information within your client record.
